Rexburg Woman Killed by Accidental Discharge During Gun Maintenance, Case Sent to Prosecutor

A 49-year-old woman died after a family member accidentally fired a weapon while performing maintenance on it inside a Madison County home near Rexburg, Idaho, authorities have confirmed. The incident occurred on June 28 at approximately 5:30 p.m., and the case has since been forwarded to the Madison County Prosecutor’s Office for review.

The victim, identified as Lesa Perrenoud, was struck by the gunshot while other family members were present in the residence. Madison County Sheriff’s Office deputies responded to the scene and immediately began life-saving efforts alongside emergency medical professionals. Perrenoud was transported by air ambulance to Eastern Idaho Regional Medical Center in Idaho Falls, where she later died from the severity of her wounds.

Circumstances of the Shooting

Investigators determined that a family member had been performing maintenance on a firearm when it discharged, striking Perrenoud. The identity of that individual has not been publicly released. As of the date authorities provided information on the case, no criminal charges had been filed. The Madison County Sheriff’s Office forwarded its findings to the prosecutor’s office, which will determine whether charges are appropriate.

The presence of other family members in the home at the time of the shooting made this an especially traumatic event for those inside the residence. Accidental firearm discharges during cleaning or maintenance remain a serious safety concern, and law enforcement agencies across Idaho regularly advise gun owners to treat all firearms as loaded and to ensure a weapon is fully unloaded before beginning any maintenance procedures.

Community Mourns a Longtime Healthcare Professional

Beyond the legal and investigative aspects of the case, the loss of Lesa Perrenoud has resonated deeply in the Rexburg community. She had served as an ICU nurse at Madison Memorial Hospital for 30 years, dedicating three decades to patient care in the Intensive Care Unit. Her colleagues and the broader medical community have lost a veteran healthcare professional whose career spanned the full arc of modern critical care nursing.

Those who knew Perrenoud described her as someone with a genuine love for the outdoors and a passion for photography — pursuits that reflected a full life outside her demanding career in healthcare. Her death leaves behind not only a grieving family but also an institution that relied on her experience and steady presence at the bedside.

The East Idaho region has seen several high-profile public safety cases in recent months. A Louisiana man was identified in connection with an ongoing search near the Snake River outside Idaho Falls, underscoring the continued work of regional law enforcement agencies on complex and sensitive cases. Locally, a Bonneville County Sheriff’s detective was recently honored for work fighting child exploitation, highlighting the dedication of law enforcement throughout the region.

What Comes Next

The Madison County Prosecutor’s Office will now review the investigative file compiled by the Sheriff’s Office and decide whether any criminal charges — such as negligent homicide or another applicable statute — will be pursued in connection with Perrenoud’s death. No timeline for that review has been publicly announced.
